The state of Tennessee has passed a new concussion law that came into effect January 1st, which is being administered by the department of health. The law does encompass club sports. Therefore we are requiring that all players/parents sign the Concussion form available at the website listed below.
It is listed as the “Parent Athlete Signature Form”

All forms (coaches and players) must be submitted to the Club President or the Coaches as soon as possible. This is MANDATORY for all players and coaches.
